Job Title Navigator
Location: Caludon Centre, University Hospital Coventry & Warwickshire
Salary: £12,486 per annum (£21,000 pro rata)
Hours: 22 hours per week
Purpose of the job:

To work alongside Ward Managers and Clinicians within Coventry and Warwickshire Partnership Trust’s Caludon Centre in a minimum of two wards initially.  The Navigator will ensure that patients admitted onto the wards and those receiving treatment are assessed to identify whether the patient has support needs that can be met by Voluntary Sector services.  The Navigator will work with the patient to ensure a seamless service from the point of admission to post discharge facilitating access to such services and enabling them the best chance to recover.

Essential Criteria:

  • Proven track record of carrying out a range of assessments (initial, formative and summative).
  • Experience of working with a wide range of agencies.
  • Experience of working for a Third Sector mental health service provider.
  • Experience of carrying out/being responsible for a caseload and working with service users/patients in a mentoring/support role.
  • Experience of undertaking monitoring and evaluation.
  • A minimum of two years’ experience of working with adults with mental health issues.
  • Ability to work on own initiative.
  • Excellent communication skills, both verbal and written.
  • Knowledge of local services
