Coventry and Warwickshire Mind has been funded to deliver a Community Wellbeing Service that will involve working alongside managers and clinicians within Primary Care Networks (PCNs) at GP surgeries.  This new and evolving service provision will provide community wellbeing and signposting support services to adults across Coventry.

This exciting opportunity will offer the post holder the chance to be part of an evolving service delivery, providing and developing a range of support interventions to people with varying mental health needs, to enable them to access community support services.  They will also be part of a wider team of mixed, experienced professionals from clinical and non-clinical support backgrounds and services and part of an established, reputable and expanding local (and national) organisation offering both generic and specialist mental health and wellbeing support services.

We are looking for individuals who are flexible and approachable and have extensive experience of working within an organisation providing a range of support activities to vulnerable adults with varying mental health needs.

We would welcome applications at NVQ Level 3 or equivalent experience in one or more of the following: Social Work/Youth Work/Psychology/Mental Health/Occupational Therapy/Community Development/Education.

The aim of the service will be:

  • To work alongside managers and clinicians within Primary Care Networks at GP surgeries to reduce pressure on GP appointments and prevent people from escalating into secondary mental health care services.
  • To support patients with non-medical, socio-economic needs and include advice, guidance, signposting to local service support provisions including, community groups, voluntary sector and statutory services.
  • To contribute towards improving personal and community resilience and reducing health inequalities by addressing the wider impact on health and providing support with practical needs such as debt, housing and physical inactivity.

Key Responsibilities:

  • To oversee a staffing team working across PCNs in Coventry and Warwickshire.
  • Work closely with the Service Manager and identified PCN Leads to help shape and monitor the service delivery ensuring that any presenting concerns around day-to-day activities are constructively resolved in a timely manner.
  • Work with PCN Leads to ensure that all referrals allocated are appropriate for the service and that staff are working to full capacity and appropriately managing individual patients accepted in the service.
  • Carry a fixed caseload working with the more complex patients identified.
  • Support staff in team and individual meetings to monitor best working practices and the effectiveness of the service, ensuring that the range of support activities and interventions used are of a high standard and appropriate to patients emerging practical and emotional needs.
  • Be committed to making the project work, through role modelling best practise behaviours and enabling CW Mind staff to do the same so that they can positively participate and contribute to the wider PCN community wellbeing/social prescribing model service.

Essential Criteria Includes:

  • Experience of supporting in the delivery of mental health support services, including crisis intervention.
  • Experience of working with a wide range of agencies.
  • Skilled in developing and delivering a person-centred approach to assess, plan and review an individuals’ needs.
  • An ability to communicate effectively both orally and in the written form.
  • An ability to manage conflicting priorities and one’s own time with a minimum of supervision.
  • Excellent ability to maintain professional boundaries.
  • Up to date knowledge of mental health.
  • Awareness of the issues around working with vulnerable adults.
  • An ability to work from across a number of CW Mind and community venues.
  • Min. NVQ Level 3 or equivalent.
  • Flexible approach to working evenings and weekends.
